As part of the District's Covid-19 response, a Health Check form has been developed that all employees working onsite in District buildings are required to fill out at the start of their shift.
The form will display in a web browser window when any employee or staff member logs in on their District Windows or Mac computer. To complete the form, employees will need to log in with their AD credentials and answer some COVID-19 symptom-related questions. If the form does not display, a shortcut with the title "Health Check" will be provided on the desktop, or employees can visit https://health-check.ccsd.net/.
